  • Review this week’s Learning Resources and consider the insights they provide about impulse-control and conduct disorders.
  • Select a patient (13 years old SA that I saw on September 15, 2023 for recurrent shoplifting for no apparent reason) for whom you conducted psychotherapy for an impulse control or conduct disorder during the last 6 weeks.
  • Impulse-Control Disorder: Below is the clinical notes of our encounter on September 15, 2023
    • Problem: SA is a 13 years old African American male who was brought to the clinic by his mother with complaints of recurrent shoplifting for no apparent reason. His mother explains that SA was recently released from juvenile jail after he stole some video games DVDs from a video their neighborhood store.  This is a recurrent behavior for SA and it has happened 9X in the past 13 months. His mother stated that the case manager involved in this case has brokered a deal with police and state prosecutors to have him evaluated by a mental healthcare professional for any possible behavioral problem. He was referred to the clinic by his psychiatrist for psychotherapy treatment. His mother is very troubled by SA’s behavior and needs help for him.
    •  Intervention: With support from my preceptor, and I discussed SA’s recurrent shoplifting behavior. I tried to identify the causes of his recurrent shoplifting incident and asked him to list any recent shoplifting episode that he can remember and why he keeps shoplifting? SA revealed to me that resist the urge to shoplift and that when he sees something in the store that he likes, he just go for it.
    • I discussed with my preceptor, possible best course of action to help SA deal with his recurrent shoplifting behavior, and she proposed implementing cognitive-behavioral therapy (CBT) to help him identify and challenge his distorted thought patterns and develop coping strategies to manage his impulsive urges. She also recommended implementing and teaching SA and his mother the exposure and response prevention (ERP) technique, which is a gradual exposure to situations that trigger the urge to steal so that SA can practice resisting the urge while utilizing some prevention coping strategies.
    • Response: SA was interested in learning the exposure and response prevention (ERP) technique and he was hopeful about his future and to participate in his next therapy sessions. In response to his urges to shoplifting, he agreed to try the exposure and response prevention (ERP) technique at home and make some changes to his urges of stealing.
    • Plan: I will keep working with SA and his mother to help him deal with his stealing impulses. In the coming weeks, I will also talk about more ways for SA to deal with his stealing impulses and to improve his mental health status as a whole.
  • Create a Comprehensive Psychiatric Evaluation Note on this patient using the template provided in the Learning Resources.
