HPI: The I.N. is a 14-year-old African-American male patient that presents to the clinic today with his mother and younger brother at the bedside. He states that he has been having some nasal congestion that has been going on for one week. He reports that he is out of his allergy medications and needs to come in for a well-visit to get a refill on his prescriptions. The patient also reports that he has been experiencing some pain in his left knee when playing football. He reports that he hears a “clicking” noise and that the left knee is tender to touch “at times.” He states that this started one month ago. He denies any difficulty with mobility and states that the pain is relieved “on it’s on.” He states that he mostly has the pain when he jumps rating that pain a five on severity.

This is the symptom analysis section of your note. Thorough documentation in this section is essential for patient care, coding, and billing analysis. He lives at home with his mother and his younger brother. He reports that he is in the 9th grade in school. He plays football and has hopes and dreams of one day “making it big.” The patient enjoys hanging out with his friends and playing the PlayStation online when he is not busy with school work or playing football. The patient states that his grandmother is a big help to him and his younger brother when his mother is at work or unable to provide what is needed at the time. He denies any use of alcohol or tobacco. Include occupation and major hobbies, family status, tobacco and alcohol use (previous and current use), and any other pertinent data. Always add some health promotion questions here, such as whether they use seat belts all the time or whether they have working smoke detectors in the house, the condition of the living environment, text/cell phone use while driving, and support systems available.
Fam Hx: Illnesses with possible genetic predisposition, contagious illnesses, or chronic illnesses. The reason for death of any deceased first-degree relatives should be included. Include parents, grandparents, siblings, and children. Include grandchildren if pertinent.
Surgical Hx: Denies any surgical history

Mental Hx:
Denies anxiety.
Denies depression.
Denies suicidal thoughts or ideations or the thoughts to harm self.
Violence Hx: Denies any violence in the home or being bullied at school.
Reproductive Hx: Patient denies being sexually active at this time. Menstrual history (date of last menstrual period [LMP]), pregnant (yes or no), nursing/lactating (yes or no), contraceptive use (method used), types of intercourse (oral, anal, vaginal, other), and any sexual concerns.
ROS: This covers all body systems that may help you include or rule out a differential diagnosis. You should list each system as follows: General: Head: EENT: and so forth. You should list these in bullet format and document the systems in order from head to toe.
