Elements of Evaluation:

Element One: “The Family”: Provide a brief overview of the family interviewed – structure of family, ages of members, stage of family development, geographic area of residence, educational level obtained, career and job information, and other such data.

 

Element Two: “The Illness”

Describe the chronic illness.

Describe basic genetic basis/pathophysiology for the disease.

Explain role(s) of the CURRENT health care providers for this specific older adult – primary and specialty care.

Explain disease and treatment strategies currently being used by this specific older adult and family and compare to the usual textbook treatments.

Discuss any advances in treatment related to the genetic basis of the disease.

Element Three: “The Older adult”

Address the impact of the illness on this specific older adult’s physical and activity needs.

Address the impact of illness on this older adult’s psychosocial, cognitive, and emotional developmental needs.

Use 1- 2 quotes from the older adult as examples in this section, if possible.

Compare Older Adult’s development to normal development using Erikson’s Theory.

Project future developmental/physical/emotional needs as the older adult progresses through the next 5 years.

 

Element Four: “Impact of illness on Family”

Discuss how the family member felt when the older adult problem was diagnosed and what they did as a response.

Describe the older adult’s/family’s beliefs related to the chronic illness, their view on the cause of the illness, and alternative and/or faith-based healing approaches used by the older adult/family.

Examine the response of friends, peers, and extended family to the older adult’s chronic illness.

Identify at least one community-based resource that would benefit this older adult and/or family. Explain the resource and provide a rationale why this resource would benefit the older adult/family.
